Behavioral Health Integration embeds mental health and substance use disorder care directly into primary care settings. Using validated screening tools like PHQ-9 and GAD-7, primary care teams identify patients with behavioral health needs, create care plans, monitor progress, and coordinate with psychiatric consultants as needed—all without requiring patients to seek separate specialty care.
BHI is available to Medicare beneficiaries who have a behavioral health condition such as depression, anxiety, PTSD, or substance use disorder identified during a primary care visit. The patient must consent to the program and have a care plan developed. BHI is designed for ongoing management, not just screening or acute crisis intervention.
BHI is billed monthly using CPT 99484 for general behavioral health integration services (approximately $48/month) or G0323 for care management services provided by a clinical pharmacist under psychiatric supervision (approximately $73/month). FQHC and RHC settings use G0511 as an alternative. All codes require at least 20 minutes of clinical staff time per calendar month.
BHI can generally be billed in the same month as RPM. Co-billing BHI with CCM requires careful documentation that distinct services are being delivered, and time cannot be double-counted.
